To schedule
an appointment, simply give us a call at (850) 939-0757. If you are a new patient
and want to save time, please click the two links below and print both the
"new patient" form and "medical history" form, fill them out and bring
them
with you.

Our current hours are Monday & Wednesday 7:00a.m to 5:00pm.;
Tuesday & Thursday 7:00
a.m. to 7:00 p.m.; Friday 7:00 a.m. to
12:00 p.m. If you need to reach us after hours for an

At Ottley Smiles Dental Center,
scheduled appointments are a bond between our staff and our patients. This is our chance to spend quality
time providing the highest standard of care to each guest (and for Dr.
Jared to talk a little sports if need-be). This is the patients chance
to express needs and concerns, and to relax. It is our belief that it
is nearly impossible to express needs and concerns or to relax while
sitting in the reception room (the chairs aren't as comfortable).
Therefore, we will make it part of our mission to see each and every
patient as scheduled.
To help us be efficient with our scheduling we ask all
patients
to follow a few simple guidelines:
1) Arrive on time (5-10 minutes prior to appointment time)
2) Give 48 hours notice for cancellations
3) Allow for longer appointments to limit the number of times
anesthesia is administered
4) Please honor our bond. |
